A GLP-1 pen safety notice: one lot of generic semaglutide (Apotex) may display and deliver a lower dose than expected. Here is what to check and who to call.

The notice
Health Canada has published a Type II recall of one lot of the Apotex generic semaglutide pen. The affected pens may show a 0.25 mg or 0.5 mg dose in the dose window and deliver that dose in place of the expected 1 mg.
- Product: Apo-Semaglutide injection, 1 mg-dose pen (4 mg/pen), DIN 02568012, 1.34 mg/mL
- Affected lot: 7000636A
- Recall date: September 23, 2026 (Health Canada reference RA-82679, Type II, retail depth)
- Distribution: Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Ontario and Quebec
Why dose accuracy matters with GLP-1 pens
Semaglutide is started at a low dose and stepped up on a schedule, so the number in the dose window is what your clinician's plan relies on. A pen that displays and delivers a different dose than intended can leave you off that plan without your knowing it.
What to do
- Read the lot number on the pen or box.
- If it is 7000636A, talk to your pharmacist or prescriber before changing or stopping anything.
- Contact Apotex with recall questions.
- Report side effects or product complaints to Health Canada.
Nothing in the notice concerns brand-name Ozempic or Wegovy pens, or other GLP-1 drugs such as tirzepatide. It names one generic lot.
