GLP-1 Side Effects and Titration: What the First Twelve Weeks Look Like

Titration is the treatment, not a formality

The gastrointestinal effects of this drug class are strongly dose-related, and the escalation schedule exists to let your gut adapt at each step. What is expected, and roughly when

The most commonly reported effects cluster early and around each dose increase:

• Weeks 1–4, nausea is the most common complaint, often strongest in the first days after an injection and easing across the week
• Each dose increase, expect a smaller repeat of the same pattern; this is why prescribers hold a dose rather than climbing on schedule if you are struggling
• Ongoing, constipation and reflux are common and are usually manageable with fluid, fibre and timing changes

Feeling full much sooner than usual is not a side effect to be managed around — it is the mechanism working. Eating past it is the most common reason people feel unwell on these drugs.

When to stop waiting and call someone

Most of the above is expected and self-limiting. Some things are not, and the distinction matters more than any dosing tip:

• Severe, persistent abdominal pain, particularly radiating to the back
• Vomiting that prevents you from keeping fluids down
• Signs of dehydration, or a sharp reduction in urination
• Any symptom your prescriber specifically told you to report

Why the schedule affects what you pay

Titration has a financial consequence that pricing pages rarely make obvious. Brand-name GLP-1 pricing in Canada generally scales with dose, so the month-one price you were quoted is the bottom of a ramp. Generic semaglutide programs hold much closer to flat across the same ramp. Price the dose you expect to maintain, not the one you start on.

Stopping, and what happens after

Current evidence points to weight regain after discontinuation for most people, which is why Canadian obesity guidance increasingly treats these medications as long-term therapy for a chronic condition rather than a course to finish. That is a budgeting question as much as a clinical one, and it is worth settling with your prescriber before you start rather than at month nine.
